The goals of this initiative are to provide complex case consultations, systems integration, and education and training.
Systems Integration Objectives:
• Provide coordination and collaboration between systems (APD, MH, groups, community resources/services to promote improve health outcomes.
• Maintain high priority partnerships with agencies, health care providers and service providers that work with older adults and people with disabilities.
Complex Case Consultations Objectives:
• Provide technical assistance by utilizing the appropriate resources to assist agency/organization staff and/or family in managing client/consumer needs
• Coordinate and collaborate development with ongoing individuals with cross-systems needs to identify solutions
• Develop and maintain ongoing working relationships between local, regional, state agencies as well as advocates, consumers, and client/family members in order to provide the full range of appropriate services
• Maintain high priority partnerships with agencies, health care providers and service providers that work with older adults and people with disabilities
Education & Training Objectives:
• Promote access to information and knowledge relating to older adults and people with disabilities and how to provide services that meet the needs and provided in a trauma informed approach.
• Assess community needs in order to identify needs and eliminate barriers
